Vaccine Upload

Where a CCA function requires proof of vaccination, this form will allow members and their partners to upload their cards which will then be available to event organizers.

Once this need is passed, these records will be deleted.
Enter the member's name, last followed by first, to locate your online profile. The name should appear below the space. Click on it to select it.


To upload a spouse card, find the member and click "spouse" below.
